We are seeking a compassionate and dedicated Family Support Worker to join the Disability Team. The successful candidate will provide practical, emotional, and person-centred support to vulnerable children, young people, and their families, helping them access appropriate services, improve their wellbeing, and achieve positive outcomes.
Responsibilities for Family Support Worker:
· Provide direct support and intervention to vulnerable children, young people, and their families.
· Build positive, trusting relationships with families to assess their needs and deliver tailored support.
· Work with children and young people with disabilities and additional needs, promoting independence, inclusion, and wellbeing.
· Carry out home visits, family assessments, and welfare checks to identify areas where support is required.
· Develop, implement, and review individual family support plans to meet identified needs.
· Support parents and carers by providing practical guidance, emotional support, and signposting to appropriate services.
· Advocate on behalf of families to ensure they can access education, healthcare, housing, financial assistance, respite services, and community resources.
· Work collaboratively with social workers, schools, health professionals, therapists, and other multi-agency partners.
· Participate in child protection meetings, reviews, and multi-agency planning meetings where required.
· Monitor and evaluate the progress of families, adapting support plans as necessary.
· Promote safeguarding and ensure the welfare of children and vulnerable adults at all times.
· Maintain accurate, confidential, and timely case records, reports, and documentation in accordance with organisational policies.
· Carry out risk assessments and follow lone-working and health and safety procedures.
· Encourage positive parenting, family resilience, and independence through strengths-based approaches.
· Keep up to date with relevant legislation, safeguarding guidance, and best practice relating to children with disabilities.
Requirements for Family Support Worker:
· Experience of working directly with vulnerable children, young people, and families.
· Experience or a good understanding of supporting children and young people with disabilities, learning disabilities, autism, physical disabilities, or additional needs.
· Knowledge of safeguarding legislation, child protection procedures, and relevant statutory guidance.
· Experience of carrying out assessments, support planning, and reviewing outcomes.
· Ability to communicate effectively with children, families, and professionals from a range of backgrounds.
· Enhanced DBS
Working pattern: 2-3 days in the office including visits to families
